The reason they both were dress like girls was that there mother did it. She wanted for people to think she only had one kid so she only took one out at a time. Also that's why the guy seterusnya door told Tenma that there was only one child.

No there mother couldn't tell them apart they was making to mush noise, she panic and gave up the wrong kid. That's the reason she abandon Johan to fend for himself when he was 5. She wanted Anna and she accidentally gave her up, not Johan. so after she realized that, she left Johan and went to start over some where else. a new life. She just didn't care she Lost her husband ( killed sejak the rose mansion people ) then she Lost her kegemaran kid.

But the thing to really think about is that how could any mother choose between her kids. She didn't treat them equally Just like Johan saids" Live aren't created equally, live are only equal in death". and thats why Johan is so interested in lives and hes is always searching for something lebih evil more, dark than what he has gone through. Johan is just a poor Lost kid inside that need someone to understand him. He has the mind of a child he see the world in a different way. if anda really pay attention to Johan anda will see how he acts like a kid. Hes looking for a father, hes finds Tenma. Hes trying to get Anna to forgive him, just like a guilty little boy. He like to play with many things like balloons. He like to play with ants like messing up there line but instead of ants he mess with human but hes still just playing.
