I'm fat and I know it; so if someone were to ask me if I was pregnant I wouldn't feel offended. Yes, it would be embarrassing, but I wouldn't be mad. On the other hand if someone mistook me for a guy, even on the phone, I'd be kinda hurt. I wouldn't lash out at them, or even make them feel bad.
