As anda found out in the last few lines of the last chapter, the main character, Sage, is one of the yellow-eyed demon's kids/followers... whatever they are...

Theresa Miller, Doctor of Psychiatry, had an office located off the main street, in a building that also housed offices for a local insurance company and a real estate agency. It was a small office, with only the doctor's work office, examination room, a reception desk, and a play area for children.

The four Winchesters stook outside the glass door, looking inside. "This should be simple," Dean berkata sarcastically. "All we gotta do is get the watchdog away from her desk, lure Dr. Joyce Brothers out of her office, get the files, and get out before anyone gets back. Simple enough."
