Most people are assuming its WILDLIFE because the injury occurred during the time that Mariska mentioned in interviews.Remember the scene near the end of the episode where Mariska is chasing Tybor on the roof? She falls down and reaches out to prevent Tybor from falling off the roof. This may be when she landed "wrong" and collapsed her lung.
