For girls: Atashi wo namae ______
For boys: Boku wo namae ______
OR
Ore wa namae ______

Both: Watashi wa namae _______

But I probably screwed up the particles... I always have trouble with that.

anda can say : " Watashi wa <your name> desu.

anda can also say "(name) desu"

Watashi no name wa unohana desu.
