He was infecting Sam with demon blood, according to the deal he'd made with Mary 10 years previously (but she hadn't known what she was agreeing to at the time). That demon blood gave Sam access to demonic powers, starting with the visions. It was all part of the demonic plan which basically drove the story for the first 5 seasons.
