I think he'll probably will get that 'call' from the angels..

there are a few possibilities...maybe he'll use his angellic powers to save sam...he's a human and he has powers...angels may not be able to follow their own plans but maybe dean will do more with his powers..try to save sam? definitely!...all this IF he gets powers...or he'll need some other kinda help...i really think sam could be saved :)

I think he's going to get a call from the angels telling him to kill Sam and depending on what Sam does he'll either try to save him or kill him. Probably both- because I think killing Sam would be saving him too. Remember Sam made Dean promise to kill him in S2 if he ever became something he's not. Regardless of whether Sam gets the job done, he's not human anymore and I think it's only a matter of time before he does something truly evil.

Fight with the angels. But he will still try to save Sam, and I have a feeling he will make it his priority, even after what he said at the end of "When The Levee Breaks" and what he promised the angels.
